About the Position

As the Support Coordinator, you will assist people with a disability with approved NDIS plans to connect to and coordinate their informal, mainstream and funded supports. Guided by the choice of each individual, you will assist them to build their capacity to achieve greater independence and to participate more fully in their community. A key function of this role is to be able to establish a positive collaborative relationship with each participant, assisting them to develop their plan and build their capacity to self-direct their services and supports in the future.

Please note: It is a requirement that prior to commencing in this role, you must possess an Working with Children (Ochre Card),  a National Criminal History Check (within 3 months) and a current Northern Territory manual driver’s licence.
